How do I get my tax documents?

It is best to ask each office if you need to fill out tax forms for your first assignment with them. Cloud Dentistry only issues 1099s if you earn more than $600 in total from any of our offices that pay you through the Cloud Direct Pay (CDP) program. If an end-of-year 1099 is due to you for CDP, it is issued by January 31 for the previous year. If you are paid as an independent contractor by the office, they will issue the 1099 to you.

1099s from Cloud Dentistry

If you made over $600 through Cloud Direct Pay, you may receive your 1099 via email. This email will come from notifications@clouddentistry.com and will be sent to the email address on file. This may also be sent via USPS to your mailing address on file.

If you need a new 1099 resent for the current filing year, please visit the "Tax Settings" section of your account and click "Resend 1099." Please allow 1-2 business days to receive the newly requested 1099 from Cloud Dentistry.

Make sure to check your inbox, as well as the junk/spam folders, for this email.

1099's or W2's from Dental Offices

If you were paid by offices directly, they would need to send you a 1099. We advise that you reach out to them directly with any questions or concerns and to ensure that they have all of the information that is needed.

You can message the office directly from your Cloud Dentistry account, or contact the office via phone if they are no longer active.
